New Passport on Expiry of Old Passport Checklist

The checklist for the application category that you apply for will provide details on which specific forms you may need to download. In some instances, there may be more than one form.

Complete a separate checklist and application form/s for each person applying. Ensure the documents are completed accurately and signed where needed.

Form U (One per person applying)
Form I (If the new passport is made after one month on expiry of the old passport)

Points to note:-
All the details in the Application form should be as per your current passport unless you are requesting some changes in them. In case of a change, new details need to be filled in and required supporting documents need to be provided as per the checklist.
In case of a new born baby’s passport, details should be as per birth certificate and supporting documents as mentioned in the checklist.
In case of a Permanent resident, applicant will have a choice to get an Australian residence address or Indian address under question 3A after providing JP attested address proof.
In case of minor, Q28 needs to be filled in by parents by printing their name and signing.
Q29 is only for the signature / thumb impression of the applicant. In case applicant is unable to sign or provide thumb impression, Q 29 boxes need to be left blank. Parents cannot sign on behalf of the child in this column.

Confirm the passport category type you wish to apply for based on your individual situation. (New, Lost, Damaged etc,.)

Complete your application online

Step 3
Download & print the form/s, read and complete checklist

Step 4
The Ministry website may provide you with an appointment time ignore this.

Step 5
Check fees payable and payment options

Step 6
Sign form/s and checklist at all indicated places

Step 7
Ensure photos are as per specifications. Incorrect photos may be rejected by the Ministry and result in your application getting delayed.

Step 8
Check processing times to plan your travel accordingly.

Step 9
Attach all supporting documents required as per the checklist

Step 10
Submit your application based on jurisdiction by Express Post/Courier in person or through a representative (where you reside determines where you need to submit your application)

Step 11
Track your application online

Step 12
Your completed application will be returned to you by the mode of delivery selected by you once processed.
